Triage Members to the appropriate case managers for specific disease care management. Used CCMS, Amisys, Business Objects and various other programs to assess members needs for appropriate care. Also, worked on HEIDIS projects, Ohio High Risk Stratification lists, and ED Initiative programs.

Grace Hospital Fairview-April 2004-June 2006Worked as an RN in the Long Term Acute Center. Responsible for telemetry, Heparin Drips, Cardizem drips, Lidocaine drips, etc. Wound care, wound vacs, ventilator dependent patients, etc. Lutheran Hospital July 1999-2003Intensive Care Unit, Worked on the critically ill patients, assisted with procedures, ventilator care, ABG obtained, arterial line care and monitoring. CVP monitoring. Also floated to the Telemetry unit and cared for the step down patients.

Provided care to the patients at the end of life. Provided for pain relief and helped patient's family dealing with the loss. Responsible for coordinating care between STNA and nurses for end of life care of patient.

Performed home care service for the Options, Passport, Medicaid and Medicare population. Responsible for coordinating care between Home Health Aides, MDs, Physical Therapy, and Occupational Therapy. Responsible for evaluating Home Health performance and providing support to Home Health Aids.
