Medical Case Manager
CurrentCollaborate with physicians, therapy team, patients and families to establish a plan of care to maximize patient potential in the clinical setting. Liaison between patient, families, clinical team, funding resource, ECM and outside vendors to provide superior care and support to patients throughout their stay at Shepherd Center. Assist the medical team and families in making plans for discharge and referrals for follow-up appointments and services. Communicate with insurance companies, workers compensation ECM’s or financial assistance programs to secure funding for plan of care. Manage and document level of care and funding source authorizations in EPIC. Provide funding sources with medical updates as requested. Coordinate with internal and external suppliers to arrange medical equipment, home health services and supplies per physician orders and team recommendations prior to discharge. Place discharge referrals to the appropriate facility for continuation of care per physician order and interdisciplinary team discharge plan. Document in Epic for collaborative approach to plan of care and discharge planning.
