I am currently serving as a Case Coordinator Intern at Heartland Family Service, where I contribute to our mission of strengthening individuals, families, and communities through advocacy, education, counseling, and support services. In this role, I assist in managing client cases, conducting thorough assessments, and developing personalized service plans to empower individuals in need. I coordinate access to essential resources such as housing and healthcare, monitor client progress, and advocate for their comprehensive well-being. With a compassionate approach and strong organizational skills, I collaborate closely with our team to ensure effective support and positive outcomes for our clients. • Recruited approximately 15 clients through networking based on reputation & credibility • Build long-term relationships and provide personal support to clients• Provide care for up to 5 clients per week, managing multiple client schedules• Coordinate selling of houses, estate sales; plan holiday parties; perform various housekeeping, meal prepping, and decorating tasks• Transport clients to all appointments, organize bills, and run all errands as needed
