The Southwark and Lambeth Resilient Communities Programme looked beyond the traditional medical model of integrated care and supported preventative initiatives that aimed to build resilient and resourceful communities to enhance the wellbeing of local citizens. The programme had approximately 200 members, ranging from citizens, VCS and arts-based organisations to statutory commissioners and providers at all levels, who formed an informal network with the same shared ambition to effect culture change and had a desire to connect with like-minded people.The initiatives included:- Voluntary and Community Sector Development to support the sector to become more integrated and connected to the health and care system - improving access to local community based services through initiatives such as social prescribing, care navigation, health champions, directory services- promote consistency and collaborative working between patients and professionals in care and support planning- supporting organisations which provide or promote volunteering, community giving and support Southwark and Lambeth Integrated Care (SLIC) was a partnership between local GPs, King’s College Hospital, Guy’s and St Thomas’ Hospitals, the South London and Maudsley Mental Health trust, social care in both local councils, Lambeth and Southwark Clinical Commissioning groups and with local people and voluntary and community sector (VCS) groups.
