In my present job, I achieve better health outcomes by direct pharmaceutical care for geriatric patients in Community Living Center (CLC) short/long-term care facility. I conduct regular inter-disciplinary team interventions, and consultative clinical pharmacy reviews across a variety of care and complexity levels from long-term, rehabilitation, inpatient hospice, palliative care, perioperative, and ambulatory specialties in at-risk geriatric and other populations. I serve as an integral team member in optimizing pre-surgical geriatric cases to improve surgical outcome,and in patients with dementia for pharmacologic interventions focusing on safety, efficacy, efficiency, appropriateness, and longevity.Some of my proudest achievements in my present job include:● Decreased falls (FAME) and errors by partnering with geriatric interdisciplinary team on high-risk medication identification and consultations, and participating in a variety of Geriatric Research Education and Clinic Center (GRECC) project clinical demonstrations.● Boosted confidence and competency of post-graduate learners by delivering exceptional direction and guidance improving patient outcome in all levels of care.● Attained substantial annual cost savings for documented clinical interventions by utilizing innovative approaches.● Improved process efficiency by conducting active operational investigations, encompassing cognition trajectory changes over time in context of various drug-burden indices in moderate/severe cognitive impairment cohort (COACH), and impact of drug burden in operative (POSH) cohort, and rates of change in cognitive testing status and delirium rates.

I investigate diverse types of anesthesia modalities and effects of medication use within patient complexity associated outcomes and health-care utilization by creating a research database of all Veteran Health Affairs (VHA) total knee arthroplasty (TKA) cases through novel case identification methodology. I enhance field knowledge by researching disparities in TKA case outcomes, drug use, and pain assessment. I examine estimated and actual outcomes in risk and safety of types of preoperative anesthesia evaluations (face-to-face vs. video vs. telephonic) as well as identifying potential surgical procedures that may be appropriate for non-face-to-face modality.My key accomplishments during this job include:● Promoted informed decision-making by inspecting differences in TKA post-surgical outcomes, healthcare efficiencies within cataract procedures, differences in outcomes of learner vs. attending surgical procedures.● Aided integration of complementary modalities for pain including music, visual imagery, battlefield acupuncture (BFA).● Maximized personal development by acquiring knowledge in analytics, machine learning, and natural language processing with plans to implement multivariable risk assessment across surgical populations.● Led seamless execution of operations by developing database for operational and research of all national surgical patients 2000 to 2020 with extensive risk factor and outcome variable details lead to multiple and ongoing publications.

During this job, I streamlined processes by leading development of high-cost and/or high-risk pharmacotherapeutic intervention through clinical electronic ordering logistics, including adjudication of specialty and non-formulary prior authorization drug requests (PADR) according to best clinical evidence in concert with local and national guidance under scope of practice. I utilized electronic medical record consultation for generating electronic PADRs for formulary and restricted medications while optimizing PADR tracking and analysis of outcomes which impacted multiple publication robustness.My key achievements include:● Formulated best practice pharmacotherapeutic protocols and interventions with changes in evidence, formulary contracting, and logistics by liaising with chiefs of medicine and surgical specialties.● Exceeded annual pharmacy clinical cost-avoiding initiatives for 9 years by utilizing robust approaches.● Developed targeted for point-of-care reference repository for all pharmacy related resources with links installed on all local medical center desktops.● Developed a process for 9 health-systems to track, details, time efficiencies, and evaluations of electronic PADRs.

At this time period, I augmented department, health-system, and VISN 6 clinical proficiency through operational and research data coding/extraction, analysis, report building, excel template generation, dashboard creation, provider report card/detailing, and other data derived evaluation to create guidance for service level quality improvement via ongoing medication use evaluations (MUEs), operational efficiencies for clinical services, and tracking & benchmarking performance and cost comparisons across VISN and other sites with similar complexity (1a VA medical facilities, highest VA complexity designation). Established best practices through behavior change intervention targeted to healthcare and administrative colleagues including medicine specialty services to obtain clinical targets of interest, and cost-efficiency targets. My key achievements include:● Increased Pharmacy departments’ research penetration, capability, and quality, and subsequent translation to patient care impact.● Guaranteed individualized patient safety and attained population health metrics by directing large-scale pharmacotherapeutic conversions.● Decreased manual chart and administrative unstructured/document review by using proven expertise in multiple coding and analytic, survey, and web design software, as well as a keen interest in automation and efficiency.● Created data cubes to guide multiple hospital systems in assessment of case volume identification and impact potential for quality improvement and cost avoidance initiatives.
