Social Caseworker--Ongoing• Facilitated staffing with Social Caseworker—Intake to document family cases being transferred from Intake Unit to 0-3 Team in the Ongoing Unit• Facilitated Family Engagement Meetings with parents who had been placed on caseload, their attorneys, and their child(ren)’s guardian ad litems• Conducted a minimum of once-a-month home visitations to homes where the children had been placed and with parents• Documented visitations, referrals, and contacts on the state’s TRAILS Legacy and Mod systems• Conducted a minimum of twice-a-month home visitations to homes where the children who had been referred to the El Paso County’s 4th Judicial District Court’s Well Baby Court• Composed and submitted Written Status Reports for each assigned family to the supervisor and the assigned County Attorney for court review• Develop treatment plans for families on assigned caseload• Provided intensive casework services to assigned families on a caseload of 12 families• Attended court hearings and mediations at El Paso County 4th Judicial District Court• Participated in Review, Evaluate, Direct (RED) Teams to determine the disposition of referred cases Social Caseworker—Intake• Received and reviewed referrals made by the Child Abuse and Neglect Hotline for further investigation and assessment of child’s safety and risk• Conducted home visits with families and interviewed caregivers and children regarding allegations of abuse and/or neglect• Communicated with medical providers, law enforcement, and school staff regarding allegations of abuse and/or neglect• Conducted safety and risk assessments on the state computer system in TRAILS Mod• Documented visitations, referrals, and contacts on state’s TRAILS Legacy and Mod systems• Documented results of assessments and investigations as founded, inconclusive, or unfounded• Participated in RED Teams to determine the disposition of referred cases
