The Clinical Case Manager provides Intensive Case Management Services to clients who have several psychological co-morbidities that must be managed appropriately for a successful re-entry into society.

Provide rehabilitative mental health services in a clinical community setting to adults that are severely and mentally and emotionally ill that may also have challenges with substance abuse and trauma. Conducting individual and group therapy sessions. Provide crisis assessment and intervention when necessary. Collaborating with case managers, mental health providers, and healthcare providers in collaboration with client in deciding the best course of treatment.(Schizophrenia, shizoaffective, depression, anxiety, bi-polar, and substance abuse were the more prevalent challenges I dealt with daily at Exodus CRTP)
