As a licensed clinical social worker, I have over 15 years of experience in the field of social work, with a focus on medical social work and coordination of care. I hold a Certified Case Manager (CCM) credential, and I am currently pursuing a doctorate degree in social work at Capella University.Presently, I am working as a field and medical case manager in the area of workers compensation assisting in claim management. I help manage the care an employee receives after a workplace injury, while ensuring a safe return to work for the employee and cost savings for the company.Throughout Covid and shortly thereafter, I did contract work as a travel social worker providing onsite case management, care coordination, and discharge planning in various acute care settings across the US.My strengths include communicating effectively with interdisciplinary team members to advance a patient’s plan of care, identify and resolve barriers, and coordinate safe and appropriate transitions to the next level of care.
