St. Francis Hospital, The Heart Center® Critical Care Advanced Practice Practitioner coveringCritical Care Physician Assistant • Rotate coverage between Medical Intensive Care Unit (MICU), Surgical Intensive Care Unit (SICU), Intermediate Care Unit (IMCU)• Assist collaborating physicians in providing detailed history and physical examination, order diagnostic tests and labs, interpret test results, prescribe medication, generate assessment, and treatment plans.• Order and place calls for specialty consults corresponding to the patient’s condition.• Handle calls from Emergency Room for new admissions, upgrades from meg-surg floors and provide patient care in the PACU admitted to the ICUs/IMCUs.• Respond and direct Rapid Response Team (RRTs) and Cardiac Arrest Codes with critical care team and perform ACLS protocol• Respond to Neuro RRT codes and Stroke Codes in place of neuro APP team during after-hours and night shifts including performing neuro exams, NIH stroke scales, activation of inpatient stroke code protocols including placing calls to CT scan and neuro-interventionalist for CT scan results and next steps. • Management of patients on mechanical ventilations, non-invasive ventilation devices including BIPAPs/ CPAPs Skills and Procedures: Ultrasound-guided Peripherally Inserted Central Catheter (PICC) & Midline catheter placement. Ultrasound-guided Central Venous Lines in the RIJ, subclavian, or femoral veins, Shiley central venous catheter for hemodialysis, Pulmonary artery catheter (Swan-Ganz) Ultrasound-guided arterial lines, peripheral venous lines ABGs, Nasogastric tube Placement, Urinary Foley Catheterization, Fecal Management Tube placement Removal of Chest Tubes & epicardial pacing wires post-op cardiac surgeries

-Rotate between Labor & Delivery, Gynecology (surgery & consults), Clinics (High-risk, Oncology, Diabetic)-Assist in vaginal birth and C-sections-Obtain history and physical examination in OB triage-OBGYN consults in the ED with the resident on duty-Pre-round on post-op gyn patients and present patients (in SOAP note format) to the GYN resident team-Clinics duties: Cervical swabs, Pap smears, Fundal height, Fetal heart rate, gyn history

-Preround on patients in the morning-Attend grand rounds-Obtain initial psychiatric evaluation on new admits-Participant in group therapy sessions (medicine, art, recreation..etc.)-Assist the team in calling outpatient psychiatrists and patient's family/caretakers for collateral information.

-Pre-round on patients and present them to the APP gen surgery team.-Assist in general surgery (first assist, second assist), orthopedics, neurosurgery, urology, and plastic surgery cases daily.

-Obtain patient history and perform physical exam-Start IV access, blood draws-Nasopharyngeal swabs, throat swabs, throat cultures, UA, straight urine catheters-Present patient cases to the attending physicians or PAs/NPs on duty.

-Attend daily rounds with residents and attending physicians-Assist attending physician and residents in history taking and performing physical exams-Present patient cases to attending physician and residents and give assessments and plans

-Perform patient history and physical exam-Commercial driver's license physical exam-Communicate and give referrals to specialists including PT, OT, Orthopedics, Pain Management, Podiatry, Hand surgeons, Neurologists.

-Daily rounds between Medicine and ED : Perform comprehensive assessments and document findings daily in EMR -Patient presentation during rounds-Respond to patient inquiries and answer questions to and refer questions to preceptor(s) as appropriate.-Initiate necessary laboratory and diagnostic tests & interpret laboratory and test results.-Obtain consultations and referrals as appropriate.

-Obtain patient medical history & perform physical examinations-Document all patient care services in the electronic medical record.-Confer and consult with supervisor physician(s) and other providers to ensure quality of patient care-Obtain consultations and referrals as appropriate.-Responds to patient inquiries and answer questions to and refer questions to preceptor(s) as appropriate.

• Teach small group lessons for all stringed instruments, grades 5-6.• Conduct the string orchestra for school concerts twice a year.• Teach two sections of general music for grade 3.

-Teach small group lessons for all stringed instruments, grades 4-6.-Conduct the full and string orchestra for school concerts & the “Music in the Park” Festival-Conduct the Junior String Orchestra for concert in the spring.-Prepare students for NYSSMA Solo Festival (all levels).-Coach string chamber ensembles for concerts

-Teach small group lessons for all stringed instruments, grades 4-12.-Participate in the Valley Stream Community Orchestra with 4 weekly concerts & carry out librarian duties.

-Designed and implemented daily lesson plans for teaching general music.-Choreographed Orff instrumental parts for the assembly.-Organized rehearsals for the assembly.-Prepared bulletin boards.-Assisted The Metropolitan Opera field trip.

-Designed and implemented daily lesson plans for teaching and conducting string orchestra rehearsals for grades 6-8.-Conducted the 6th Grade string orchestra at the Spring Concert.-Organized and conducted rehearsals for sectionals, grades 6-8.-Assisted with chamber orchestra rehearsals, grades 6-8.-Designed and implemented a final project for the 6th Grade-Created project worksheets and final project grading rubric.-Prepared bulletin boards throughout the semester.
