Act as the primary point of contact between the patient and a broader care team. Facilitate and review Behavioral Health Screenings to understand the patient's behavioral health needs. Educate patients about their conditions, treatment options, the importance of adherence to the care plan, and leverage personal strengths and support systems in service of their health goals. Track patient progress using validated tools and participate in regular case review meetings with the collaborative care team to discuss patient progress and make necessary adjustments to their care plan.

Perform medical administration duties like answering phones, schedule appointments, checkpatients in and out, collect payments, insurance verifications, prior authorizations for medications, radiological exams, and medical day procedures and referrals. Assist multiple providers with obtaining vital signs and orthostatic levels, rooming patients and setting up for procedures, chaperone when needed, administer vaccines, electrocardiograms, check glucose levels and hemoglobin A1C levels, rapid strep swabs, collect urine cultures, assist in providing nebulizer treatments, ear lavage, staple removals and the use of electronic medical records. Other duties include stocking rooms, triaging phones calls, and educating and reviewing laboratory results with patients when appropriate.
