Current• Perform assessment and identify all problems that affect patient’s Health and wellness with an interdisciplinary team to engage patients telephonic.• Assist patients telephonically to create and implement person-centered care plans.• Ensure compliance with all clinical documentation requirements.• Complete medication reconciliation and coordinate with specialists and other providers.• Ensure closure of the referral loop.• Improve medication adherence: encouraging members with empathy and age sensitivity to build self-efficacy and lasting results.• Conduct member and provider outreach campaign initiatives to improve Medicare Star metrics by providing information and sharing best measure practices guidance.• Conduct member, pharmacy and prescriber education and outreach initiatives to improve Medicare Star adherence and CAHPS metrics. Confirm with the pharmacy the medication was dispensed and follow up with a claim number to close the gap.• Assist patients with scheduling appointments with the primary care physician and specialist
