The community-based, client-centered program develops individualized care plans for children and transitional age youth (TAY), ages 0 through 25 years. Program participants have serious emotional disturbances (SED) or severe mental illness (SMI).The dedication and diversity of the Project FOCUS team allows the program to support individuals and families utilizing a “whatever it takes” approach, and assists them to rebuild, reconnect and reach identified goals. In-home and community-based intensive case management services provide support and assistance in obtaining such things as benefits for low-income families, health insurance, housing placements, parent education, tutoring, mentoring, youth recreation and leadership development. Services are available in Vietnamese, Korean, Tagalog, Samoan, Cambodian, Chinese, Japanese, Hmong and other Asian and Pacific Islander languages. Show less
