Clinical Extern Nurse - Transitional Care - Veterans/Palliative/Community Acute Unit for Dementia and DeliriumComplex care patients: patients range from total dependence to limited assistance for ADL’sExperience in professionally handling patients with behavioral and psychological symptoms of dementia, bipolar disorders, Alzheimer's, PTSD, epilepsy, chronic pain, progressive supranuclear palsyPractices under the direct and/or indirect supervision of the Registered Nurse or Registered Practical Nurse on the unit and in a manner consistent with Sunnybrook policies standards, while taking into account CNO guidance for unregulated care providersWorks with unit nurses to ensure knowledge, skills and judgment needed to perform all actions/duties.Performs vital signs, documentation, recording oral/caloric intake, monthly VS baseline, fluid balance, etc.Assists patient with personal hygiene and ADL’s (bathing, dressing, shaving and shampooing of hair)Provides measures to ensure skin integrity and reduce pressure injuries.Applying standard interventions to support safety best practices, join in weekly Quality Management roundsSetting up patient rooms to receive admitted, transferred and post-operative patients.
