-The patient, family and caregiver are essential members of hospice care team and meeting their needs is a top priority at OneCare Hospice. -OneCare Hospice provides on-call support, patient care and patient admissions 24-hours a day, 7 days a week, including holidays. -A specialized admission team responds quickly to all patients & families. Our Admission nurses use their clinical expertise to evaluate each patient's unique needs and provide immediate care, support and education for the patient and family. - Our admission and social work staff explore all potential resources, including community services and placement (if needed), ensuring that patients and families receive individualized care to meet their long-term care goals. - OneCare Hospice is committed to community education and outreach. We take pride in sharing information, news, and OneCare Hospice services through community events and education.
