We will explore a comprehensive portfolio of your safety, accessibility needs, and home requirements within your environment. My primary objective is to gather a profile, a process aimed at gaining a profound understanding of your daily routines and activities, both within your home and daily environments.
To facilitate this interaction, we will utilize the Google Meet platform that will enable me to virtually meet with you, your family, and caregivers. This will allow me the opportunity to acquaint myself with the specific areas within your home that may benefit accessibility or modifications. The overarching goal is to collaboratively devise solutions that will empower you and your loved ones to continue residing in your home while maximizing functional independence for as long as possible. It is also crucial to emphasize that we must account for the needs of all those who live in your home and may visit your home as well.
This assessment is designed to interpret real-world problems by helping consumers avoid billions in high-cost institutionalization, facility placement, readmissions, falls, unproductive aging and provide greater independence.
The primary goal of the assessment will be to discover everything from simple assistive technology and durable medical equipment adjustments to more complex solutions to the home, with product recommendations and find local providers to work with. 90 minutes will be allotted for the initial assessment that will include a writeup with recommendations that can be provided to your clinicians and primary doctors.
If you feel that we need more time to discuss any of the process, a follow up visit can be scheduled separately. In addition, I will be available for liaison sessions with you and your clinician or provider, so the plan comes together as envisioned.
