We are the ACMA Northern California Chapter (est. 2003) We support innovative, professional development of the following: Mentoring at all career experience levels Educational Forums/CEU events and an Annual Chapter Conference Resource Information related to all levels of patient care New opportunities for expanding your professional network Influence local, state, and federal health care policies and other hot topics related to Case Management and Transitions of Care We have representation from nursing and social work case management professionals working in the Santa Cruz, Santa Clara, Stanislaus, Calaveras, Amador, San Joaquin, Alameda, Contra Costa, San Mateo, San Francisco, Marin, Solano, Sacramento, El Dorado, Placer, Sutter, Yolo, Napa, Sonoma, Lake, Colusa, Yuba, Nevada, Sierra, Plumas, Butte, Glenn, and Tehama counties.
