We offer a range of services and collaborate with over 60 partners to help our members assess their social needs and navigate available community resources, including food assistance and housing. Additionally, we work with local health home programs to provide remote and home-based care management services, improving the quality of life for those we serve.
We offer an innovative tool for seniors to easily monitor their physical and mental health parameters through our WeCare Communicator mobile app. We engage them in health seminars, promoting preventive care services, mental health, health diet, chronic disease management, etc. to reduce risks that contribute to unnecessary visits to the emergency room and/or hospitalization.
