Beacon Community Connections collaborates with social service organizations, criminal justice agencies, healthcare providers and other organizations to improve community health and well-being. The process of collaborating with these different organization is referred to as community care navigation. Community care navigation is non-clinical case management to address a person's social determinants of health. Access to housing, transportation, and food would be considered social determinants of health. Consent for contact must be obtained and a referral must be sent from a partnering organization in order to enroll a client in Community Care Navigation. Once the client is accepted, they will be contacted on a regular basis and connected with resources in the community that can improve their lives. Navigators also provide support, guidance, assistance applying for benefits, advocacy, along with many other connections. It is important to note that these services are free for our clients.
