CardioBond

CardioBond is an eCardiac rehab platform for remotely connecting post-cardiac surgery patients and care managers for cardiac rehab and secondary prevention. Using our Actionable Decision Elements (ADE) for the components of cardiac rehab and secondary prevention, our care managers are able to provide patients with a personalized cardiac rehab program. Despite all the benefits for this Class 1A treatment, 80% of post-cardiac surgery patients do not participate in clinic-based cardiac rehab due to various factors such as, lack of clinical facility access and availability, lack of transportation, and time constraints. As such, the CardioBond platform offers cardiac patients cardiac rehab outside of a clinical setting using a blend of technology and virtual visits with care managers and healthcare professionals.
