Primary care providers (PCP) manage vast majority of chronic cardiovascular conditions that constitutes a significant share of health care expenditure. Keeping these costs under control while maintaining patient's access to appropriate care is very important goal for health care organizations. Timely recommendations from a cardiologist can result in better patient outcome as well as potential cost-savings. In the current practice, a patient with cardiac disease is usually referred to a cardiologist who in-turn orders diagnostic testing done at their office or at a local hospital. This can result in significant delay in care that is often less efficient and more costly, especially in a rural setting and for practices that are part of a Value Based Care model. Cardiovascular studies such as echocardiograms, vascular studies, cardiac monitors and stress tests can be done at the primary care offices rather than referred out, if there is expertise available to have these studies interpreted. This can also result in cost savings and potentially additional revenue for PCPs Cardiovascular Web Diagnostics aim to change this paradigm by providing the following services. 1. The inter-professional Cardiology e-Consult service offered by CWD can be used by primary care practices to facilitate early non-emergent consultations with Cardiologist. 2. Our cardiologists can review patient chart at the request of PCPs to confirm the patient is on appropriate medical regimen and recommend changes to treatment as needed. 3. Remote interpretation of cardiovascular diagnostic studies done at the primary care office in a timely manner. We believe having these cardiac services readily available to PCPs can result in overall improvement in patient outcomes, at the same time resulting in significant cost savings, faster access and better patient satisfaction.
