Care Coordination Consultancy (CareCC) was established in response to local community demand for quality disability support services.
CareCC Support Coordination team specialises in Psychosocial Recovery, Support Coordination and Specialist Support Coordination.
A small team of dedicated school teachers at CareCC passionalte about emplyment and education provide School Leaver Education Supports and specialise in supporting youth involved in the justice system's.
The whole team work closely with Particpants and navigate the sectors many systems such as Forensic, Justice, Health and Mental Health, including liaising with the Public Guardian, Child Safety, Quality Safeguard Commission and NDIA.
Our vision is to change lives through service delivery were people come first, by empowering wellness, healthy minds, connection, hope and choice.

CCC complies with all Queensland and Commonwealth Government requirements for the delivery of quality and safe disability support services.
