Care Management Solutions, LLC was born of a passion for working with seniors and advocating for individuals struggling with psychosocial and health care situations they find confusing, upsetting, frightening or incomprehensible. We are committed to honoring our clients and families by giving them a voice and enriching their lives. Care Management Solutions, LLC provides exceptional social work and care management services to seniors and adults with disabilities to enable them to be happy, healthy, safe and live in the least restrictive environment. We also provide assertive health care advocacy services to adults of any age, either as part of or separate from, ongoing care management services. Care Management Solutions, LLC is available to give presentations geared to to professionals or the public on health care issues, community resources, caregiving, advance directives and advance care planning and a variety of other topics.
