Our care coordination software is designed to address the pressing issue of delayed discharges in the NHS. Our primary goal is to facilitate efficient and effective communication and coordination among healthcare providers, patients, and caregivers, ultimately leading to smoother transitions of care and reduction of the length of hospital stays. Delayed discharges result in a higher incidence of hospital-acquired infections, reduced bed availability for emergency department admissions, also referred to as "bed blocking," and impose a substantial financial burden on the National Health Service (NHS) and taxpayers, amounting to as much as £820 million annually.
