CareNeighbor

CareNeighbor is a community health worker patient panel management system. We provide knowledge and tools to help community health workers efficiently guide patients to existing disease management programs, local resources, social services – and ultimately better health. And we help community health workers strengthen existing trusted relationships and improve health with the support of technology. CareNeighbor enables consistent and measurable community health worker interventions, and we support the technology that helps make these workers scalable. We help people seamlessly access existing resources, connect with their social networks and improve their health literacy to build individual capacity to achieve wellness.
