Collaborative action and research for engagement, persistence and adherence in treatment & health, (CarePath) is a pioneering EU-funded project under the Innovative Health Initiative (IHI), bringing together a multidisciplinary consortium of partners across healthcare, research, and technology. This research-based, public-private partnership is dedicated to improving treatment adherence and persistence for people living with chronic diseases, recognising these as critical drivers of better health outcomes and more sustainable healthcare systems.
By integrating clinical innovation with digital health solutions, CarePath is developing and testing person-centred care pathways that address the complex and varied needs of individuals managing long-term conditions. This includes co-creating innovative support tools and strategies that empower patients, caregivers, and healthcare professionals to make informed decisions and sustain effective treatment over time.
The project places a strong emphasis on health equity, aiming to reduce disparities in access and quality of care, and ensure that digital tools are inclusive, accessible, and adaptable across different European contexts.
