Case Management Cymru delivers person-centred specialist case management to adults and children with acquired brain injury, birth injuries, learning difficulties, spinal or orthopaedic injuries, mental health issues, including PTSD and older adults with diminished capacity.
From our base in Rhondda Cynon Taff (RCT) we provide services to individuals in South and West Wales and beyond.
At CMC we understand Welsh health and social care services and can adapt to the variety of socio-economic communities that exist across the region.
We also draw upon a wide local network of professionals and services, enabling us to work effectively and collaboratively across sectors.
