CHAP’s mission is to create stronger community health systems through strategic collaboration and catalytic investments. CHAP works at the state and national levels to create locally driven, network supported solutions for systems change. Our aim is to reconnect health by shifting care from a strictly medical approach to one that harnesses the resilience of communities. We support policies and programs to sustain a Community-Based Workforce, with an emphasis on birth equity. Our unique role is serving as connective tissue among public, private, and community leaders.
