Welcome to The Collaborative Healthcare Network. Our vision is one of an interprofessional collaborative model to provide patient-centered healthcare. When multiple healthcare providers from different professional backgrounds provide comprehensive services by working with patients, their families, caregivers, communities, and other health workers , new possibilities exist that were not there before. The Collaborative Healthcare Network is a diverse multidisciplinary team of health specialists focused on restoring health, function, and wellness in an effective, efficient, and collaborative manner to our community. Modern health care is embracing collaborative care as multiple research has demonstrated that it strengthens health-care systems and improve patient outcomes. When a person's health and wellness are the priority we acknowledge that one healthcare professional may not provide all the answers. CHC Network takes into account a comprehensive perspective of health and wellness that each discipline is an integral cornerstone to the positive outcome of a person's health. Our collaboration is not simply team work. As we work as an interprofessional team of various backgrounds and training we strive to value RESPECT as it is essential to the success of shared leadership, overcoming barriers to collaboration, eliminating harmful stereotypes, and ultimately creating a more patient-centered approach to health-care delivery.
