Instead of addressing cardiovascular, diabetes, and kidney conditions or risk factors separately, the Cardiometabolic Center Alliance has taken a novel approach to treating these comorbidities together—providing team-based, comprehensive care specifically tailored for each patient.
The Cardiometabolic Center Alliance provides its members with the proper training, protocols, and resources needed to establish their own Centers of Excellence. As the Alliance enhances members' ability to measure outcomes, improve processes, and expand research opportunities on a national scale, the ultimate benefit is improved patient outcomes.
By expanding this clinical model of care, our vision to help patients better manage their conditions for a better quality of life and to ease the burden on healthcare organizations will become a reality.
