Community ConneXor

A technology-enabled social enterprise building connected community networks of social care organizations to improve the health and well-being of vulnerable individuals. Our community-up model has two pillars; our social care management technology platform, Community Resource Marketplace® , and complementing community referral network development services. Community Resource Marketplace® has five key modules; Assessment and Screening, Resource Directory, Referral Management, Care Coordination and Reporting and Analytics. Community Referral Network development services complement and optimize the technology platform to engage in closed-loop digital referrals. We partner with our customer social service organization (HUB) to recruit and onboard its referral partners (SPOKES), other social service organizations they refer clients to for services.
