About Community Care Partners

Community Care Partners (CCP) is a collaboration of two community-based providers, Bay Cove Human Services and Vinfen Corporation, that offers integrated care coordination to MassHealth enrollees who have complex medical and behavioral health needs. Our multidisciplinary Care Teams partner with enrollees to overcome barriers and meet their health and wellness goals while working collaboratively with healthcare providers and community based social services providers to better meet enrollee needs.
