Founded in 1913, Cornerstone VNA is a nonprofit home, health and hospice care organization serving New Hampshire and Maine. Our mission is to promote the optimum level of well-being, independence and dignity of those living in the community by providing trusted, compassionate and expert health care. Cornerstone VNA places great value on transforming the home health care experience and is proud to provide care for all ages, from birth though end of life. Our skilled team offers the highest quality nursing, rehabilitative therapies, social work, volunteer and support services through five distinct programs: Home Care, Hospice Care, Palliative Care, Life Care-Private Duty, and Community Care.
