The philosophy of Crescent Home Health Agency is simple: “Bringing Living Back to Life.” Since opening our first office in Swansea, Illinois in 2010, the sole focus of Crescent has been two-fold. First and foremost, Crescent is focused on improving the quality of life for our patients, through our home health services. The second part of our focus is improving the community in which we live, through education and prevention. For our patients, Crescent’s professional staff provides the highest quality of care in the safe and familiar environment of their home. For our community, Crescent provides educational programs for local civic groups, nonprofit organizations, service groups, and businesses. It is the mission of Crescent Home Health Agency to provide individuals with the information necessary to help reduce the risk of injury or illness and lead a healthy and active life.
