Digital Health Transformation Service

We are an NHS-based project team supporting other NHS healthcare teams with well-evidenced digital tools to enable service users to access reliable, trusted health & wellbeing advice and information. We go further to support our NHS and healthcare providers at every step, making it as easy as possible for their teams to embrace and deliver change. We do this because we believe it empowers service users, fosters interaction and improves early intervention, especially for the most vulnerable of service users. ChatHealth is a risk-managed messaging helpline platform, providing a way for service users to easily and anonymously get in touch with a healthcare professional. HealthWebsites are a suite of age-appropriate public health information websites (Health for Teens, Health for Kids and Health for Under 5s) that provide service users with a unique localised experience alongside national content. HealthForms is an online health needs assessment tool for improving service user outcomes. HealthGuidance is an online portal that delivers targeted health content to service users at the point of referral.
