Divinity Chronic Care Management (DCCM) is a subsidiary of Divinity Services. We have established this unit to offer support, guidance, and care to patients diagnosed with chronic condition/(s).
Our aim is to help individuals diagnosed with chronic condition/(s) improve their quality of life by providing them guidance on how to manage their chronic condition(s) as well as guiding their primary care providers.
Our Nurses provide care coordination and remote patient monitoring to help patients bridge care gaps and reach optimum. Our dedicated team of qualified and experienced nurses will help the patients:
1. Understand the risks and benefits of each treatment related to the chronic condition(s),
2. Side effects of the medicines/treatment to help them make an informed decision regarding their health care,
3. Respond to frequently asked health-related questions,
4. Guide patient on regular basis on complications that can arise from the chronic condition(s) and how to avoid them
5. Maintain, update and manage the patient’s health record and share it with doctors, emergency responders, or caregivers when needed.
6. Schedule or reschedule appointments for the patient.
