ElevatingHOME’s mission is to align, unify, and strengthen the home-based care industry. We envision home-based care providers as an integral and critical component of high-quality health care delivery models. ElevatingHOME advances the principles across the care continuum that health care starts at home, that the home is a cost-effective setting for care delivery, and those outcomes are often best at home. Formed by the leadership of the Visiting Nurse Associations of America (VNAA), with leaders and stakeholders across the country, ElevatingHOME is a nonprofit created to unify and represent the 12,000 home health agencies and 6,000 hospice organizations in the United States. ElevatingHOME was built to advance and strengthen the entire home-based care industry and serve as its voice in the health care system.
