Experienced Support Coordination, LLC assists participants in gaining access to needed programs and services, as well as needed medical, social, educational and other services. Our Support Coordinators are responsible for developing and completing the NJ Individualized Service Plan (NJISP), and maintaining ongoing contact to assure services are provided to the individual's satisfaction. Our team of Support Coordinators pairs out-of-the-box thinking with service knowledge. ESC Staff recognize that each individual and family circumstance is unique and will strive to provide you with the assistance you need to find the services you want - to live the life you want. We will strategize with you to creatively utilize your budget and other community and natural resources in order for you to achieve and experience the best possible outcomes.
