Five Rivers Care was founded in 2023 by community likeminded people who live in the Peel region and have been employed in the disability and healthcare sectors for many years. The name Five Rivers Care comes from the Kimberly region of Western Australia where the rivers (Ord, Forrest, King, Durack, and Pentecost) join and enter the Cambridge Gulf. The concept of these rivers coming together in the Cambridge Gulf – for us at Five Rivers Care, is similar to the holistic care model of people relying on various services to meet their individual care needs.
