Five Star Physician Services provides innovative clinical leadership to acute, sub-acute, rehabilitative and palliative-care institutions through the use of Acute and Post-Acute Care. Our group is based out of Maryland and our medical professionals provide high quality patient care and create effective partnerships with hospitals and facilities. Five Star was formed with the vision of creating an effective clinical link between acute/post-acute physician services.
Five Star is a geriatric clinical care practice comprised of Physicians and Nurse Practitioners exclusively serving skilled nursing, rehabilitation, long term care, assisted living and CCRC communities located in Northern Virginia, Maryland, and Southern Delaware. In several facilities that we serve, we provide Medical Directorships, Attending Physician, and Nurse Practitioner care, allowing us to take a leading role in quality patient care and improving quality care ratings in the facility.
