Flourish Holistic Services Pty is a registered and approved NDIS Provider, we also provide assistance to participants who are referred by DHHS, TAC, WORK SAFE, WORK COVER AND TO MEMBERS OF VICTORIA. Our qualified Support Coordinators who are qualified Social workers are keen to assist with your needs as per your goals. During the process of intake, our qualified Social Worker will assist you in creating a support plan as per your required needs, short term goals and long term goals. Our Allied Health practitioners are keen to support you with your therapy, Recommendation of Assistive technology to assist your with your daily life and provide you final report after providing 10 sessions of therapy or as per your required needs. Our well experienced Support worker are keen to support you with community participation, social activities, assist in providing support in high intensity Care and Personal Care. We work in the best interest of participants in promoting their needs to achieve their goals in long term. Our well experienced cleaners will assist you in detailed cleaning and maintaining your home environment to maintain hygiene and cleanliness. At Flourish Holistic Services we provide assistance to High Care Needs, Complex behaviours, low care needs and mental health care. We have experience in Autism Spectrum Disorder, Intellectual disability, Physical Disability, Acquired Brain Injury, Vision impaired, Deaf and difficulty in hearing and Mental Health Conditions.
