Great Lakes Home Healthcare, House Calls & Clinical Physical Therapy is Traverse City’s choice for the complete continuum of rehabilitative care. Locally owned and operated since 2003, we provide fully licensed expert, one-on-one services in skilled nursing, physical, occupational and speech therapies, medical social work and home health aide care.
As Medicare certified providers, our nurses and therapists work closely with physicians and primary care providers to care for the home-bound patient.
Once the patient has improved enough to graduate from home-bound status, our House Call division can seamlessly continue therapy in the home using the same therapists with whom the patient is comfortable. Specifically, House Calls is available to those seeking the convenience of therapy at home, yet aren't home bound.
In addition, individualized out-patient therapy is offered at each of our 3 clinics.
