Grounded Stars is a culturally grounded mental health and wellness initiative that centers youth of color while remaining fully accessible to all young people from birth through age 24. We lead with one bold principle: prevention is power. While based in New York, our virtual services reach youth and young adults nationwide. As a prequalified New York City Health and Human Services provider, we deliver trauma-informed care, life skills education, and culturally affirming mental health support to youth and families navigating complex emotional, social, and systemic challenges. We offer early intervention, youth-centered therapy, mentorship, and creative wellness programming—designed to help young people build emotional resilience, community connection, and real-world coping skills before crisis begins. We proudly deliver a growing range of Medicaid-eligible case management and clinical services, including: Targeted Case Management for youth involved in justice, reentry, or court-ordered services Health Home Care Management for youth with serious emotional disturbance or chronic health needs Family Wraparound Support coordinating care across education, housing, legal, and health systems Crisis Prevention, Referrals & Resource Navigation Administrative and Non-Medicaid Support Services for underinsured families What We Offer: Individual & Group Therapy (Ages 0–24) Social-Emotional Learning (SEL) Courses Creative Wellness Workshops & Peer-Led Projects Academic Support & IEP/504 Advocacy Life Skills Coaching & Job Readiness Programs Family Support & Case Management Peer Groups, Game Nights & Cultural Programming Community Events, Sport Challenges & Youth Art Projects We create safe, engaging spaces where youth feel seen, supported, and celebrated.
