Hang Tough was founded to improve the quality of life of patients and families living with multiple health disorders.
The foundation aims to help build a new, integrated model of care and ignite meaningful research that enables open collaboration.
An initiative was borne out of the belief that patients with multiple health issues require seamless and coordinated treatment. Each member of a patient’s medical team must understand the complexity of health issues and focus on providing an integrated care strategy that identifies risk factors for unintended consequences of therapies, disease prevention, and best practices for procedures that deliver optimal patient outcomes.
The initiative sets out to develop a new model of care, led by a world-class team of physicians and researchers from UHN. Through proactive collaboration and dialogue, this team will untangle the complex interactions and consequences of disease treatments to improve the prognosis for patients burdened by multiple life-threatening diseases.
