We are a community-based, integrated healthcare team dedicated to serving adults with intellectual disability and their caregivers. Services: 1. Clinic Services - We provide comprehensive health services in a welcoming environment, with subsidized consultations to ensure professional care for patients and caregivers. 2. Home Visits - After an initial in-clinic assessment, the IDHealth team can make home visits when necessary, providing island-wide service to ensure everyone is reachable. 3. Dr. Connect - We have an established network of primary care partners to refer patients with intellectual disabilities for less complex health needs, such as health screenings and management of acute and chronic conditions. 4. Professional Support - We collaborate with other health and social service providers to address the diverse needs of people with intellectual disabilities in Singapore, offering resources, training, and support to strengthen our community together.
