RN, Certified Diabetes Care & Education Specialist (CDCES), & Nationally Board Certified Health & Wellness Coach (NBC-HWC) providing chronic disease education and support with a focus on Type 2 and Pre-Diabetes. I've worked with a number of primary care clinics providing case management and health coaching since 2019. As contracts with providers are constantly changing and benefits through insurance varies from year to year, private practice provides an opportunity for existing clients to continue the coaching relationship as well as welcome in new individuals to serve.
