HCR has been providing care management services through New York's Medicaid Health Home program since 2013. In 2016 HCR launched HCR Care Management, LLC, in support of this growing business. Care Managers oversee and provide access to services for patients enrolled in the program. We are continually looking for additional Care Managers and other support staff as we continue to expand this business.
