Honor My Decisions is a healthcare technology company that provides cloud-based enterprise advance care planning (ACP) software, education, and training. Our fully-featured set of ACP tools and associated services have been developed by a team of experienced healthcare, technology, education, and user experience professionals. Our products synthesize bioethicist research, results from peer-reviewed ACP studies, and best practices in consumer engagement and technology innovation to solve the key problems with paper-based ACP. Our end-to-end ACP ecosystem is designed to help family doctors, healthcare systems, health plans, and employers quickly launch and grow this important service population-wide or enhance and expand existing ACP programs. Our ACP application is relevant and beneficial across the lifespan: from healthy adults to those with serious illnesses as well as people at end-of-life. We support a variety of delivery models and channels to suit every need. Our ACP can be completed independently or with the assistance of a facilitator, either in person or virtually. Telehealth ACP can be easily and seamlessly delivered by existing staff in chronic care management, disease management, complex case management, and other programs. In-clinic ACP can support health providers – from care managers to network physicians – in facilitating, documenting, and earning reimbursement for ACP. Our ACP education and engagement can be delivered via email, website, SMS, health plan portal, smart assistant, and other channels. We have digital legal advance directive forms for all 50 states as well as a video living will. Our secure cloud-based repository stores each person's advance care plan at a unique URL and is available 24/7. It includes all elements of their comprehensive HMD ACP: emergency contacts, healthcare decision-maker, treatment preferences, legal documentation, end-of-life values, and their video living will.
