Hope PACE

The mission of Hope PACE is to provide client-centered holistic long term care to frail elderly individuals while supporting their core desires for dignity, autonomy, and the ability to maintain a degree of independence. We’re different from any other program or service. We focus on care at home instead of going to a nursing home or other care facility. When you enroll in Hope PACE, we handle everything you need—your primary and specialty medical care, medications, homecare, socialization and so much more, all designed to help keep you living safely and independently in your home.
