The HWFC Action Learning Lab is a research/ innovation incubator group dedicated to the growth of communities that cooperate and innovate to promote the physical, mental, social, spiritual health and well-being of people. We facilitate Canadian-made solutions in research, technology innovation, education/training, knowledge mobilization (policy) and community activation to spin off to receptor groups. We use a public health approach that includes:
1. Systematic social health risk screening
2. Person-directed, goal-oriented, and quality of life focused assessment, care planning and community navigation interventions and prevention-focused outreach
3. Wellness-oriented group programs (public education, peer support, intergenerational)
4. Use of person-reported quality of life assessment, outcome, and experience measures to improve social health, quality of life and reduce unmet needs.
5. Student learning internships and mobilization of intergenerational volunteers
The goal is to build a culture of health and wellness by identifying upstream and downstream opportunities and addressing them within and outside formal systems. By attending to physical, mental, social and spiritual health, everyone benefits. By partnering with traditionally underserved communities, health inequities are reduced. By building capacity in people and communities, a social movement is initiated.
Our Lab offers virtual opportunities including Quality Life Phone Support, Social Health Education, Wellness Support, and Outreach. We also support program development within interested organizations.
