By promoting best practices: Identifying strategies across the patient journey in six priority areas, from health literacy to screening high-risk populations, integrated care pathways, self-management support and workforce participation.
Testing innovative solutions in 142 pilots in 18 European countries to improve CVD and DM prevention, management and patient empowerment.
Using a standardised 15-step methodology to facilitate analysis of success factors and contextual influences.
Promoting broad communication: Sharing our experiences and results benefits other countries and stakeholders across the European Union (EU).
