Welcome to the JACARDI LinkedIn page! We are a collaborative initiative of 21 European countries committed to tackling the challenge of cardiovascular disease and diabetes. Our mission: to reduce the burden of these diseases and their associated risk factors at both individual and societal levels. We're committed to promoting health systems that are sustainable and equitable for all. How do we achieve this? By promoting best practices: Identifying strategies across the patient journey in six priority areas, from health literacy to screening high-risk populations, integrated care pathways, self-management support and workforce participation. Testing innovative solutions in 142 pilots in 18 European countries to improve CVD and DM prevention, management and patient empowerment. Using a standardised 15-step methodology to facilitate analysis of success factors and contextual influences.
